The numbers on the dashboard were treated as ground truth — the way dashboard numbers usually are. Nobody was auditing the customer feedback data feeding into it, because the top-line sales figure looked healthy, and a healthy top-line number rarely triggers anyone to go looking for a problem underneath it.
